USE OF COMPOSITE BONE GRAFT IN LONG BONE FRACTURE HEALING STABILISED BY INTRAMEDULLARY PINNING IN CANINES

Abstract
The present study was conducted on 14 clinical cases of dogs with diaphysial fractures of Humerus, Femur and Tibia/Fibula. Based on the technique used, the animals were divided into two groups. In group I fractures in 8 dogs were repaired with IM pins along with cerclage wires wherever required. In group II fractures in 6 dogs were stabilized by using intra-medullary pinning and a composite graft of calcium hydroxyapatite and fresh autologous bone marrow aspirate was applied at the fracture site Pre-operative clinical and radiographic examination was performed in all the cases. Orthopaedic surgeries were assisted by intraoperative fluoroscopy wherever required. The process of fracture healing was assessed on the basis of clinical and radiographic examination performed during the follow up period. Clinically, lameness was graded in to 5 degrees from 0 to 4 in increasing order of severity on the basis of amount of weight bearing on the affected limb during station and walking and pain at fracture site. Radiographically, the process of healing was graded in 5 grades from 1 to 5 in decreasing order of healing on the basis of amount of callus formation and visibility of fracture line on post-operative radiographs. At 60th postoperative day functional outcome in all cases was determined and graded as excellent, good, fair and poor. Dogs in group II started partial weight bearing earlier than other groups with a mean 3.33 ± 0.61 days. Least lameness was recorded at 30th and 60th post-operative day in dogs of group II. At the time of complete weight bearing, radiographic evaluation revealed best healing in dogs of group II. In 102 group I fracture repair occurred with minimal callus formation. Functional outcome was excellent in 10 cases, good in 3 cases and fair in 1 case. The complications recorded in group I were IM pin migration in case A7 and swelling at fracture site in cases A3 and A5 and group II , seroma formation at the level of trochanteric fossa in A2 and A5 Complications recorded in group II were seroma formation B and,B3. wound dehiscence and pin migrationin case B1. Implants were removed in selected cases on satisfactory performance of the affected limbwith complete healing of fracture supported by radiographic examination.in some cases implants were removed due to the development of post-operative complications.
